Just enter the following forms completely and accurately. Once all information has
been completed you will review the CREDIT APPLICATION AND MEMBER BUYING GROUP AGREEMENT
at which time you can print the complete application and FAX or mail it to The Combine
Buying group for approval.
You will immediately be emailed your username and password for use within the Combine
Buying Group website. You may browse, compare products and services as well as view
any information about Combine. Orders will not be processed until your application
has been signed, dated, received and approved.
You will need the following information available to completely fill out the application:
- * Federal Taxpayer ID#:
- * DUNS #
- * Owner, Partner, Corporate officer, and guarantor Information
- * Credit & Trade References
- * Bank Information
